    Issues of clinical relevance with child rearing: The genetic contribution: Heredity makes a significant contribution to intellect and also to other qualities such as tem­perament and personality, but the exact extent of the genetic contribution is controversial.

For instance, very wide variation in environmental stimulation in a particular population would increase the environ­mental effect. The genetic influence is not con­fined to the effect on total IQ scores.

    Pathological influences on intelligence Intrauterine and postnatal pathological factors such as malnutrition must be regarded as environ­mental influences on brain development, and therefore on neurological and intellectual func­tion. Pathological factors which affect the physical structure and growth of the nervous system could be responsible both for developmental disorders and for at least a part of normal variation. For example, some developmental problems might result from subtle perinatal brain damage, which could be either focal or diffuse. The literature relating brain disorders to problems of intellect, learning and behaviour is diverse and confusing. Clearly, the relationships are very complex. It is naive and misleading to attribute developmental disorder to ill-defined 'brain damage' without considering what this means.

    Environmental influences: In some circumstances intelligence is clearly related to environmental factors. For example, Rutter noted that 'there is abundant evidence that mild mental retardation is extremely common among children brought up in city slums. Intelli­gence develops and is not a "given" capacity. Its development is a social process strictly dependent upon the quality and organisation of the human environment.'

Head injury studies support the idea that, in children, brain injury is more likely to result in a global intellectual deficit than in a specific developmental or learning disability.     10 If one excludes cases of identifiable perinatal brain damage associated with obvious neurological abnormality in the neonatal period, some relationship still exists between perinatal disorder and intellectual and learning deficits, but the correlations are weak and it is far from certain that the relationship is causal. In studying such relationships it is vital to control for the numerous psychosocial factors which contribute both to the occurrence of perinatal disorder (e.g. prematurity) and to the outcome.

    The concepts of interaction and resilience

    The concepts of interaction and resilience: The development of any child is determined not by genetic or environmental factors in isolation but by the interaction between them. The child's temperament and abilities affect the parents' responses to the child — in other words, the child contributes to the creation of his/her own en­vironment.

    Parental responses and interactions are also influenced by the extent to which the child matches their own preconceived ideas about the kind of child they hoped to have. This has been called the 'goodness of fit' between parental expectations and the child's actual temperament and personality.

    All children are exposed to some degree of stress and adversity, but they vary in their resilience or vulnerability in response to life experiences. The differences are partly dependent on genetic factors such as temperament but are also affected by protective factors in the environment.

    Assessment

    Too many people 'assessment' has come to mean administration of a developmental or psycho­logical test, but this is only a small part of the assessment process, and in many cases it may even be irrelevant to the presenting complaint. Assessment has been defined as 'the systematic collection, organization and interpretation of information about a person and their situation'. It should be conducted for a purpose rather than as a ritual. A very different approach is needed in each of these situations and it is vital to identify and deal with the real problem — for example, a conflict over management.

    A complete assessment might include the fol­lowing components: definition of the main problem(s); evaluation of the genetic, biological, environmental and pathological factors which contribute to the problem; an estimate of the child's present development status — this may vary from a rough approximation to a detailed set of measurements using a psychometric test battery; tests of hearing and vision; evaluation by speech therapist, physiotherapist or occupational therapist; the organization of an intervention programme; and a review of the response to intervention.

    Methods of assessment

    For the paediatrician, the usual approach would be an interview with the parents, followed by involvement of the child in simple play and/or observation of their spontaneous activities. The information obtained in this way is interpreted using a developmental scale, together with one's general knowledge of child development. A devel­opmental scale is simply a means of arranging information about the mean ages at which children achieve various milestones. The way in which information is elicited about the items in the scale is left to the discretion of the clinician, and the extent of normal variation is indicated only in the most general terms.. Items are classified into convenient, though arbitrary, categories, for example: (i) posture and large movements; (ii) vision and fine movement; (iii) hearing and speech; and (iv) social behaviour and play.

    There are many clinical situations in which it might be sufficient for the paediatrician to state, for example, that a child aged 3 years is function­ing at the level of an 18 month old. Parents find it easy to understand this concept and can state whether or not they agree with the doctor's estimate. This information can also be expressed as a developmental quotient:

    However, this practice has some disadvantages; it has an aura of accuracy which is not justified by the precision of the test procedures used, it conceals variations between different aspects of development, and it is mistaken by parents and sometimes other professionals for an IQ. and is interpreted accordingly.

    Using parents as a source of information

    Using parents as a source of information: There is a widespread belief that, to parents, 'all their geese are swans', and that therefore their report of the child's abilities is likely to be hope­lessly exaggerated and quite unreliable. As a result, excessive reliance is placed on the small and often unrepresentative sample of behaviour which can be elicited during a brief assessment session, and the parents' vast store of knowledge about their child is ignored.

    Yet only the parents can know whether the child's performance is representative of his/her true abilities or can explain the history of individual items of behaviour — why, for example, a word is used in a particular way or why the child shows an un­expected reaction to a task. Furthermore, the way in which they describe the child and his/her activities gives some insight into the quality of the child's microenvironment.

    There are several reasons for the apparent unreliability of developmental information obtained from parents. Firstly, retrospective recall of the times at which milestones were achieved in the past is known to be unreliable. Secondly, where the child's current abilities are concerned, the questions put to the parents are often am­biguous and their replies are not subjected to further critical probing.

    Thirdly, it is usually the parent`s interpretation of what they observe rather than their observations which is unreliable; resent­ment is generated by the failure of doctors and other professionals to recognize this distinction. Agreement must be established first with the parents on what the child can and cannot do, with specific examples being cited. Only then can the significance of the child's abilities be discussed.

    Occasionally, parental accounts do seem to substantially over- or underestimate a child's abilities, in comparison with what is seen during assessment. This discrepancy is itself of consider­able diagnostic importance, for the parents' lack of understanding may explain many management problems and might be a contributing cause in some developmental disorders. We have seen a small number of parents whose self-deception about their child's abilities is so extreme that it takes on the quality of a delusional system.

    A few parents are more consciously manipulative and their description is distorted in order to obtain the opinion they desire. Even more rarely, they may deliberately try to mislead the doctor, perhaps for medicolegal reasons. All of these situations are uncommon, but they must be recognized if the family is to be helped. The parents' view of the child's problem is every bit as important as an 'objective' assessment of the child's development.

    Psychological tests I

    Many doctors regard assessments based on inter­view and observation as crude and inaccurate, and feel that 'proper' tests, using more structured procedures, are essential for diagnostic precision. It is therefore worth noting that psychologists define a test as 'any systematic procedure for observing a person's behaviour and describing it with the aid of a numerical scale or a category system'.

    A systematic interview or an observation session constitutes a test in terms of this definition. The important question to ask is not4 Is this test accurate enough?' but 'Will this procedure provide reliable information relevant to the problem in hand?'

    When it does seem necessary to quantify one's observations accurately, a formal test procedure may be used. For example, it may be useful to decide whether a deaf child has normal intelli­gence when confronted with non-verbal tasks, or to assess how much a child's IQ has changed following a brain injury.

    The mechanics of test administration are not difficult to master; the skill lies in the interpretation of the results, which calls for a knowledge of the psychological literature. For this reason, the supply of many test kits is restricted to qualified clinical or educational psychologists, who will normally be responsible for the detailed assessment of these more complex problems.

    Psychological tests are designed to make systematic observations and quantify them. The results are then related to normative data; in other words, they are compared either with the individual's own previous performance or, more often, with the results obtained by a group of similar individuals. The first of these approaches is useful in, for example, planned teaching pro­grammes for children with learning disabilities, as a means of evaluating progress. The majority of the psychometric tests used in clinical assessment are of the second type, and the most familiar are those used to measure intelligence (IQ tests) and language development. Procedures are also available for the examination of personality, behaviour, brain damage, and many other factors. An important distinction is made between tests of IQ and tests of attainment, such as reading or writing.

    Many psychometric tests are standardized. This means that they require a specified set of materials, the instructions and mode of presen­tation are precisely stated and the results are assessed by comparison with the known per­formance of normal children from a comparable background. The tester may of course use their discretion to depart from the specified procedure, but the more this is done, the less accurate the result will be.

    Psychological tests II

    New tests are checked for reliability, to ensure that comparable results would be obtained by different observers or on different occasions. The most important characteristic of a test procedure is its validity.

    Validity is concerned with questions such as: what is this test actually measuring? does this test give a true measure of the child's abilities in this particular area? what deductions can be made from the results? and do these results imply that the child has a problem which can be expected to interfere with future progress and prospects?

    In general, any assessment of a young child should be regarded primarily as a measure of current functioning, a method of quantifying what the child has learnt up to the present moment. Prediction of future progress or ultimate intel­lectual potential from measures made in early childhood is unwise, unless the child's abilities are repeatedly found to be in the severely learning-disabled range. Many factors contribute to the difficulties of prediction, including the intrinsic deficiencies of the test; variations in the skill of the tester; anxiety, shyness, lack of motivation or undiagnosed hearing loss in the child; and unusual patterns or temporary setbacks in devel­opment.

    The tester has to make a subjective judgement of the extent to which these factors might invalidate the results. Also, since the child's ultimate abilities depend to some extent on experi­ences yet to come, it is intrinsically unlikely that one could predict their future on the basis of their performance in the first few years of life.

    Several studies have shown that by 2Vi years of age there begins to be some correlation between the test scores and later intelligence. The ability of an IQ. score to predict academic and career progress increases throughout childhood. In the majority of children IQ_ is a surprisingly stable quantity; significant and occasionally dramatic changes can occur, but these are exceptional. Furthermore, it is not possible to predict how a particular life event or experience will affect the IQ of an individual child. Most children will tend to seek activities and environments in which they feel most comfortable, so that their intellectual powers are not encouraged to grow; but a few may deliberately engage in tasks which stretch their capacity and perhaps increase their IQ as a result.

    Tests available to doctors

    The Griffiths test is the most readily available formal test for the paediatrician. It is a standardized scale for chil­dren aged 0—8 years. It requires a special test kit, which is only supplied when a training period has been completed. The results are presented as a developmental profile. The doctor who can use only this test is at a definite disadvantage compared with the psychologist, who can select the most appropriate procedure from an extensive repertoire. Furthermore, some aspects of this test are open to criticism on technical grounds. In particular, the construction of the test does not allow easy separation of language from non-verbal items.

    The Nature of Disability

    The Nature of Disability: Tolstoy opened his novel Anna Karenina with the provocative observation that 'Happy families are all alike; every unhappy family is unhappy in its own way.' He might have gone on to remark that every family with a disabled member is disadvan­taged in its own way.

    Some families cope well with the most distressing problems, whereas others are devastated by disabilities that in purely medical terms are far less severe. The differences are to be found in the personalities and life experiences of the child and their parents, the functioning and strength of the family unit and the effectiveness of their network of support among relatives, friends and professional services.

    The problems of the disabled child are often complex and multiple. No single profession can have the requisite knowledge and skills to deal with all of them. For example, the diagnosis and management of a child with a language disorder or learning disability might involve a speech therapist, psychologist and teacher. In these disci­plines there is less emphasis on, or expectation of, a unitary diagnosis and more emphasis on defining problems that are amenable to intervention; in fact, there are many situations in which the precise medical diagnosis is of little relevance and is of considerably more interest to the doctor than to the child, their family or their teacher! An increasingly vocal body of opinion, particularly those working with learning-disabled adults, believes that disability is not primarily a medical problem and that treating it as such merely creates handicap.

    When parents are told that their child is disabled, their perceptions of the child and of themselves are changed. There is still a stigma attached to disability. Goffman (1963) in his classic study described the concept of stigma thus:

    “the Greeks, who were apparently strong on visual aids, originated the term stigma to refer to bodily signs designed to expose something unusual and bad about the moral status of the signifier. The signs were cut or burnt into the body and advertised that the bearer was a slave, a criminal, a traitor — a blemished person, ritually polluted, to be avoided, especially in public places ... today the term is widely used in something like the original literal sense, but is applied more to the disgrace itself than to the bodily evidence of it.”

    Parents may reject any medical term which implies disability or handicap; it seems that medical labels and therapy may stigmatize the child as much as their actual disability. An opposite phenomenon may also occur; parents may seek eagerly for a medical label, since our society has come to believe that modern medicine is powerful enough to have an answer for all problems if they can be defined in medical terms.
